Retreat Registration and Agreement

Please read the below agreement and sign at the bottom to acknowledge that you understand and agree to the terms and conditions.

1. Acceptance of Contract: 
Upon purchase of the "Connect to Your Heart" retreat at Sacred Waters Center for Restoration and Retreat in Hood Canal, WA, hereinafter referred to as ‘the retreat’, the terms and conditions set forth herein become a binding contract between the purchaser and Astrid Pujari, MD and Ann Strickland, LMHCA. Acceptance of this contract by the purchaser shall occur when payment in full, referenced in Section 3 below, is received by Astrid Pujari, MD.

2. Definitions: 
The retreat will consist of chanting and meditation practices, lodging, and meals, beginning at 5:30 PM on Friday, January 2, 2026, and ending on January 4, 2026, at 11:00 AM. The retreat does not include expenses for travel, tips for staff, or any food, accommodations, travel, etc. outside of the specified dates and times. The purchaser must arrive at the designated start point at 5:30 PM on January 2, 2026; this is when the retreat begins.

Further, the purchaser acknowledges that, in organizing and leading the package, Astrid Pujari, MD, and Ann Strickland, LMHCA, are serving in the capacity as retreat organizers but not as practitioners of medical arts and that they shall not undertake any obligation to diagnose or treat any medical or health conditions or mental health conditions which may arise, or injuries which may be incurred by the purchaser during the retreat

3. Reservation and Payment: 
The purchaser understands and agrees to the following: 

• Registration and payment in full of $495.00 is due by December 15, 2025. The cost for a single room is an additional $125.00; however the number of single rooms is limited and will be filled on a first-come, first-served basis. 

• The cost of the package is non-refundable and non-transferrable once paid, regardless of circumstances.

• To register for the retreat both this signed contract and the non-refundable payment in full must be received by the office of Astrid Pujari, MD no later than December 15, 2024. Failure to submit one or both registration or payment prior to December 15, 2025, precludes registration, regardless of expressed intention.

4. Participation: 
By purchasing this package, the purchaser guarantees that this retreat is appropriate for their health, physical abilities and interests. Astrid Pujari, MD and Ann Strickland, LMHCA reserve the right to accept or decline to retain any person as a participant for any reason that they determine. Should they feel the participation of any one person may affect the operation of the retreat, the rights, welfare and/or enjoyment of the other participants, and/or the health and well-being of the purchaser, they may cancel that person’s reservation in the exercise of their sole discretion. Should the purchaser decline participation in any activity or element of the retreat, there will be no refund of any portion of the package cost.

5. Confidentiality:
The purchaser agrees that: 

• Any information they share during the retreat is at their sole discretion, and it is not guaranteed that information they share during the retreat will not be shared by another participant outside of the retreat in violation of this agreement.

• They will not share any information regarding another participant outside of the retreat without that participant's explicit permission, and the purchaser will take full responsibility for any resulting actions should they violate this agreement.

• They will not share harmful or malicious information nor take any disruptive action or actions during this retreat that could negatively impact other people, within or outside of the retreat, or that could negatively impact the retreat.

6. Warranty Disclaimer: 
No guarantee or warranties, expressed or implied, are made that the package will meet all of the participants’ expectations. Astrid Pujari, MD and Ann Strickland, LMHCA reserve the right to make cancellations and/or make changes in services where such changes become necessary by circumstances beyond the control of Astrid Pujari, MD and Ann Strickland, LMHCA and/or all affiliates. Astrid Pujari, MD and Ann Strickland, LMHCA reserve the right to make changes to the package itinerary based on unforeseen conditions and situational events including, but not limited to travel delays, crime, inclement weather, civil disruptions and/or other potential sources of harm, as well as for purposes of enriching the participants’ experience. If changes are made to the retreat, the cost of retreat remains non-refundable. 

7. General Disclaimer and RELEASE: 
I agree that Astrid Pujari, MD, Ann Strickland, LMHCA, and all affiliates are released and held harmless from any and all liability while participating in any activity within the retreat including, but not limited to: travel delays/problems, weather and other acts of God, illness or emotional harm, accident, injury, death, loss of or damage to personal property and loss of income. This disclaimer and RELEASE are given knowingly and voluntarily and with a full understanding of the terms thereof. 

8. Communicable Disease Disclaimer and RELEASE Agreement: 
I agree that Astrid Pujari, MD, Ann Strickland, LMHCA, and all affiliates:

• will not be held responsible for any exposure to any communicable diseases, including any variant of the Covid virus, nor held responsible for any medical complications or expenses in the event that the purchaser contracts a communicable disease, including any treatments for same. This includes the possible spread of any a communicable disease to any other person the purchaser has contact with during the retreat or otherwise. 

• will not issue refunds should Covid or other communicable disease cause travel delays, itinerary changes, cancellations, or any other unforeseen disruptions occur.

• assumes that the purchaser will at all times make a good faith effort to protect themselves and all persons they come in contact with from contracting and/or spreading communicable disease by leaving or not attending the retreat if they have been diagnosed with a communicable disease or have symptoms of such.

• Masks are NOT required at this retreat, and in signing this contract, Astrid Pujari, MD, Ann Strickland, LMHCA, and all affiliates are released and held harmless from any and all liability from participating in the retreat, including the contracting of all communicable diseases including Covid. All participants are responsible for their own medical expenses, Covid related or otherwise.

9. Entire Agreement:
The foregoing are the complete terms and conditions related to purchase of the package and participation in the package retreat and there have been no other statements made, or information provided upon which a participant shall be deemed to have relied in electing to purchase the package and participate in the package retreat. Please provide the requested information and signature below. By signing this contract, you signify that you have read and understood all information therein, and agree to all terms, including all RELEASES. Failure to complete this agreement prior to November 30, 2026, will nullify your participation eligibility and no refund of deposit will be provided.
