By signing this agreement, I agree to its terms and conditions and confirm I understand that:

 

• I am entering into an agreement with Astrid Pujari, MD for a Yagya monthly membership

  • the monthly tuition for the Yagya membership is $168.00, paid month-to-month.

  • $168.00 will be automatically charged to the credit card I have on file on the first day of each month and will continue unless I cancel my membership

  • I may end my participation at any time. My card will be charged on the first day of the month. If I give notice of cancellation within the first 7 days of the month, then I will be charged one more month’s membership before termination of membership fee. If I give notice within days 8-31 of the month, then I will be charged 2 more months’ membership before termination of membership fee.

 

• The schedule to be set and emailed to me prior to the beginning of each month along with a Zoom link

 

• I am responsible for knowing the date and time and having the correct Zoom link. I understand that I will not be refunded for any classes that I do not attend, regardless of circumstances. 

 

• there will not be a video or audio recording of any class.

 

• the class may be canceled or rescheduled by Dr. Pujari with advance notice; in the event of cancellation, there will be no make-up class.

 

• this class is offered by Zoom only and I am solely responsible for ensuring that I have a working Zoom account, that I either know or will learn how to use Zoom, and that I will ensure my Zoom is working prior to the class.

 

• this class is intended for spiritual education only and is not intended to diagnose or treat any illness or provide any medical advice, diagnosis, or treatment. I will hold Astrid Pujari, MD harmless for any information provided.

 

• the dynamic of the group lends to an open and intimate sharing of personal information, and I can share or not share within my level of trust and comfort, without expectation.

 

• Confidentiality is of the utmost importance, and I agree to not share personal details that are shared by others during class with anyone outside of the group.​
